Spouse Enrollment Code: Your own FULL LAST NAME (caps)+ 4 digit Employee # + SP (SP stands for Spouse)
***Each User needs their own separate email address Username.
***No spaces or hyphens in Last Name

Examples:
Employee BRAD SMITH: SMITH1234
Spouse JANE NELSON: NELSON1234SP
